What this site is for
This site collects expressions of interest from clinicians, practices, and organizations in a planned paid beta. Submitting the form does not create an account, purchase, or guarantee of access.
What we collect
When you submit the interest form, we receive everything you enter. Name, email, organization, state, program tier, provider count, and website are required. Everything else is optional, but if you answer a question, we receive your answer:
- Contact details: professional name, business email address, and phone number (optional)
- Organization: practice or organization name, state or region, your role, and organization type
- Program interest: the tier you are interested in and your approximate provider count
- Optional add-on interest: whether you are also interested in the optional clinical governance add-on
- Practice profile: care settings you serve, populations you serve, and the EHR or EMR system you use (including the name of the system if you choose "Other")
- How you work today: how intake is done now, the number of new clients per month, who would review risk alerts, and when you could start a pilot
- Your website: your practice website, or LinkedIn page if you do not have one
- How you found us: your answer to "How did you hear about us?"
- Your goals: what you would like to evaluate, in your own words
- Your confirmation that we may contact you about the program
The form also records, automatically, where your visit came from: the tracking tags in the link you clicked (for example, a LinkedIn post) or the website that referred you, or "direct" if neither applies. We use this only to learn which outreach is working.
Our hosting provider may also keep routine technical records, such as IP addresses and browser details, to deliver and protect the site. We do not add advertising trackers or analytics tools to this site. If we add privacy-friendly analytics in the future, we will update this notice first.
These questions describe your practice, not your clients. Please keep answers general and do not include client or patient information.
Please do not send patient information
This site is for professionals. Do not submit protected health information, patient names, clinical narratives, or other sensitive records through the form or by email. The planned beta is described as using synthetic data only.
How we use your information
- To read and respond to your interest
- To discuss whether the program is a fit and which tier applies
- To send you an automatic email confirming we received your submission
- To contact you about the founding partner program
We do not sell your information.
Who else handles it
We use service providers to run this site. Netlify hosts the site and stores form submissions for us. Resend delivers the automatic confirmation email to you, and our email provider (Proton Mail) receives the form notifications and our replies. Each handles your information only as needed to provide that service.
How we protect it
This site is served over HTTPS, so what you submit is encrypted in transit. Form submissions are stored with Netlify and are available to us and to the service providers described above. No online service is completely secure, so please share only the professional information the form asks for.
How long we keep it
We keep interest submissions only as long as needed to respond to you and evaluate the program. You can ask us to delete your submission at any time.
Your choices
To see, correct, or delete what you submitted, or to stop us contacting you, email Safebridge.ai@proton.me from the address you used on the form.
Your privacy rights
Depending on where you live, including California, you may have the right to know what personal information we hold about you, to correct or delete it, and to opt out of its sale or sharing. We do not sell your personal information or share it for advertising. We will respond to these requests from anyone who contacts us at the address above.
Changes
If we change this notice, we will update the date above.
